Service Description

Medical evaluation requested by a third party, such as an employer or insurance company or for other personal reasons. This includes adoption medical exams, pre-employment exams, employment exams, exams to join a sport team and exams to enter an education program. Our professionals provide thorough assessments to meet specific requirements. We do onsite audiology testing (audiograms), occupational health forms, vision including ishihara 24 testing, Epworth sleepiness Scale, Respiratory Surveillance Questionnaire and other screening forms as requested. We organize X-rays. The radiology department directly bills your company. We organize/refer off-site spirometry for pulmonary function assessments. The pulmonary forms and billing for this service will be directly sent to you from their department. We don't offer onsite lab work. We do onsite 14 parameter urine dip analysis. Please sent all forms and assessments that are required to us before the appointment. Note: we do not provide assessments to determine if an employee is fit to return to work after an injury, WSCIB exams, or medical-legal assessments. Contact us for the fee if you are a new client or company. The fee is dependent on the type of exam and forms required.
